The 2015-16 4K and Kindergarten registration for Union Grove Elementary School is Tuesday, Jan. 6; Wednesday, Jan. 7; and Thursday, Jan. 8, from 8 a.m. to 3:30 p.m. at the school main office, 1745 Milldrum St., Union Grove.
4K registration
• At registration you will have needed to complete the online registration or turn in a paper copy.
• Paperwork for registration can be completed online ahead of time from website www.uges.k12.wi.us under the Parents Tab/4K Registration or you can stop in the main office of the school to pick up a packet.
• The child who is being registered must be four years of age by Sept. 1 of the current year.
• Parents must bring proof of residency, their child’s birth certificate, and immunization record for verification.
• It is not necessary to bring the child being registered with you to registration.
Kindergarten registration
• Screening will not be done at registration time.
• At registration, you will have needed to complete the online registration forms or turn in a paper copy.
• Paperwork for registration can be completed online ahead of time from website www.uges.k12.wi.us under the Parent/Kindergarten Registration or stop in the main office at school to pick up a packet.
• The child being registered must be five years old by Sept. 1, 2015.
• Parents must bring proof of residency, their child’s birth certificate, and immunization record for verification.
• After all documents and registration forms have been verified, you can sign up for an appointment for Kindergarten screening.
• It is not necessary to bring the child being screened to registration.
These registration dates for Kindergarten registration replace the dates previously scheduled in February.
